Joint Providership Accreditation

Joint Provider (your organization) must complete the required activity documentation to qualify for accredited CME designation.

This form must be completed and returned to director@uoma.org. The information provided by your organization will allow the UOMA to review the proposed CME activity for possible accredited CME designation.

This form is used to identify and mitigate all potential conflicts of interests that arise from financial relationships with commercial interest(s) of CME planners with control of activity content.

All joint providers must submit a planning activity application and review fee. In addition, the joint sponsor is charged an overall joint providership fee based on the number of credits for the activity. See Joint Providership Fee Schedule.

Review the Professional Practice Gap & Educational Need section. Complete the Learning Objectives by focusing on core competencies.

Financial Relationship Disclosures must be must be completed, signed and collected from each contributor to the education; and then, summarized onto this form.
